In a recent interview, Kevin Boehnke, Ph.D., research investigator in the department of anesthesiology and the Chronic Pain and Fatigue Research Center at the University of Michigan, shared his thoughts on the THC, CBD and pain relief management matter. Boehnke explained,

“It seems like if you take them together, you can tolerate more THC without having the same types of side effects. Also, taking CBD and THC together, compared to a similar amount of THC alone or CBD alone, seems to have better analgesic effects.”

There have been several reviews that have analyzed the way that pain can be impacted by cannabis in general, which would ordinarily be helpful to the everyday consumer. In the same year, another report came out, but from the National Academies of Sciences, Engineering, and Medicine (NASEM). This report stated that cannabis’s ability to treat chronic pain was supported by “conclusive or substantial evidence.” This research added that nausea and vomiting (associated with chemotherapy) and muscle spams and stiffness (related to multiple sclerosis) were also verifiable. Covering research published through the last 18 years before the study, the pain seemed to find the greatest soothing from cannabis, though the report added details about the impact on the risk of cancer, cardiometabolic issues, and respiratory disease. It also stated that cannabis use can be listed as “better performance on learning or memory tasks.”

Cochrane Library published another study, examining the use of cannabis for the treatment of chronic neuropathic pain. The study offered another contradiction, saying that “there is a lack of good evidence that any cannabis-derived product works for any chronic neuropathic pain.” However, it is important to note that there are other pain conditions that the researchers believe should be studied to examine the reaction of the body with cannabis.

The Problems With Current Studies

Even with the variations in the reports, one common element is the limits that previous studies have shown. Boehnke points out that many of the studies are “small,” and they don’t often provide the participants with long-term use of cannabis, which means that they are incapable of determining “exact underlying pain phenotype.” More so, Boehnke stated, “[T]hey don't use cannabis products that are representative of what people are using today.”

Presently, there’s a major roadblock by the Drug Enforcement Administration that keeps researchers from adequately accessing legal marijuana for research. The only authorized supplier for the DEA is the University of Mississippi, even though the authority has stated that it was planning to accept applications for others before seemingly being halted with the Trump administration. If the quality was the right quality, this wouldn’t be a concern. However, researchers have stated that the marijuana provided by the university is low in quality, doesn’t have the genetic profile of marijuana (as it is more like hemp), and sometimes contains mold, posing a huge risk.

Safety and Efficacy

So, is CBD safe? The answer isn’t clear since studies are so widespread and inconsistent. However, considering the quality of cannabis that is being tested, that could be the culprit. CBD itself is likely to have more reliable studies since it is now legal to perform the necessary testing on it.

Does CBD work? Perhaps, but there is simply not enough research to be conclusive. A study published in Pain considered the effects of CBD in animals, discovering interactions with serotonin receptors. These receptors, releasing the chemical that makes the individual feel elated and relaxed, are believed to play a role in pain, though they are also linked to depression and anxiety. Still, Boehnke points out, “It’s just tough to know because CBD is such a promiscuous molecule.”

In a review published in 2017, authors in the British Journal of Clinical Pharmacology found conclusive evidence that CBD had a significant impact on the spread of cancer. Though it did not solidify the eradication of cancer cells, CBD was found to prevent the spread of cancerous cells but also promote their destruction.
